The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

The goals of HFNY are to promote positive parent-child interaction; to ensure optimal pre-natal care; to promote healthy childhood growth and development; and to enhance family functioning.
Mothers participating in the HFNY study were significantly less likely to deliver low-birth-weight babies than mothers in the control group (3.3% vs 8.3%). HFNY parents also reported having engaged in significantly fewer acts of serious abuse and neglect.

The mission of the Healthy Kids Express Asthma Program is to provide quality asthma care and education to children with asthma and their caretakers in a school environment to work towards goals set by Healthy People 2020 and the National Asthma Education and Prevention Program (NAEPP).
The Healthy Kids Express Asthma Program serves approximately 600 children a year, improving inhaler technique and asthma knowledge among participants. Children enrolled in the program for two consecutive years have lower school absentee rates and hospitalizations due to asthma.

The goal of Healthy Relationships is to reduce the risk of HIV transmission by people who know they are HIV-positive by reducing the frequency with which they have unprotected sex.
Healthy Relationships seeks to develop decision making and problem making skills to reduce the risk and transmission of HIV through behavioral intervention. An intervention study resulted in significantly less unprotected intercourse and greater condom use with lower estimated HIV rates post-intervention.

The Pennsylvania Department of Aging offers this fall risk screening and prevention program to adults 50 years of age and older. The program is designed to raise awareness of falls, introduce steps on how to reduce falls, improve overall health, and provide referrals and resources.

Improve early identification of concerns and initiate interventions to improve the health, development and emotional wellness of children, newborn to age three.
HSFYC parents were less likely to use severe discipline (OR: 0.68) and more likely to negotiate with their child (OR: 1.20). HSFYC parents had greater odds of reporting a clinical or borderline concern regarding their child's behavior (OR: 1.35).

HMG National supports affiliate states by:
Promoting development and expansion of a national network of states that are building HMG systems
Providing technical assistance to help states implement the core components and structural requirements of HMG
Informing the public discourse on the crucial importance of optimal child development
Ultimately, HMG National's goal is for all families to have knowledge of and easy access to statewide systems that support them in learning about their children's developmental needs and finding appropriate services.

The goal of HELPS Programs is to strengthen students’ reading fluency so they will be better able to focus on and improve other important reading skills, including comprehension.
HELPS is a supplemental curriculum that improves students reading fluency, a commonly neglected aspect of children's core reading curriculum, in order to help them become successful readers.
